On today’s episode of Dr. M’s Women and Children First Podcast, we welcome a scientist whose work has quietly shaped the cardiovascular health of millions around the world.

Dr. Sundeep Dugar is a pharmaceutical innovator, inventor, and industry leader with more than three decades at the forefront of drug discovery. He is best known as a co-inventor of ezetimibe — marketed as Zetia® — a landmark cholesterol-lowering medication that transformed lipid management by targeting intestinal cholesterol absorption. He also co-inventor of the combination therapy Vytorin® (ezetimibe plus simvastatin), expanding treatment options for patients at high cardiovascular risk. For this groundbreaking work, Dr. Dugar and his colleagues received the prestigious 2005 National Inventor of the Year Award from the Intellectual Property Owners Association and the Heroes of Chemistry award from the American Chemical Society. Across his career, Dr. Dugar has contributed to more than 140 patents and has authored over 70 scientific publications, reflecting a lifetime devoted to translating chemistry into real-world therapies.

He is currently the founder of Aayam Therapeutics, where he leads efforts to develop innovative, accessible medicines through collaborative global research. He also serves as Co-Chief Executive Officer of Blue Oak Nutraceuticals, advancing a novel mitochondrial-targeted compound known as Mitokatlyst™, designed to stimulate mitochondrial biogenesis and cellular energy — with potential implications for muscle strength, metabolic health, cardiovascular function, and inflammation. He is the first one to decipher the mechanism by which exercise induces mitochondria levels. Mitokatlyst mechanism of action mimics this process.

Dr. Dugar’s scientific journey spans continents and some of the world’s premier institutions. He earned both his Bachelor’s and Master’s degrees in Organic Chemistry from the University of Delhi, completed his PhD in Chemistry at the University of California, Davis, and pursued postdoctoral research at ETH Zürich in Switzerland and at Cornell University.

Let’s dive into articles that have been critical to understanding type 2 diabetes and Insulin Resistance: the Ominous Octet and now the Egregious Eleven from the BMC Medicine paper and the 2009 Diabetes paper by Dr. Ralph DeFronzo. Peter Attia recently interviewed Dr. Defronzo and I am on listen #3 because it is so good and yet incredibly complicated. This piece is a compilation of that interview and reviewing Dr. DeFronzo’s papers.

“…all too often, the risk factors for these disorders are not addressed promptly in clinical practice, leading to irreversible pathologic progression… Traditional approaches to treatment involving sequential therapy, in which agents are added only after one has failed, contribute to clinical inertia and often prevent goal attainment, leading to adverse outcomes…. in turn contributing to increased morbidity and mortality. In contrast, early diagnosis and prompt, intensive intervention, often with initial combination therapy, leads to faster goal attainment and improved outcomes for at-risk patients.” (Handelsman et al. 2023)(Schwartz et. al. 2024)

David L. Katz, MD, MPH is a specialist in Preventive Medicine and Lifestyle Medicine, with particular expertise in nutrition.
He earned his BA at Dartmouth College (1984); his MD at the Albert Einstein College of Medicine (1988); and his MPH from the Yale University School of Public Health (1993). He completed sequential residency training and board certification in Internal Medicine (1991) and Preventive Medicine/Public Health (1993). Katz is the founder and former director of Yale University’s Yale-Griffin Prevention Research Center (1998-2019); Past President of the American College of Lifestyle Medicine; President and Founder of the non-profit True Health Initiative; and Founder and CEO of Diet ID, Inc. The recipient of numerous awards for teaching, writing, and contributions to public health, Katz was a 2019 James Beard Foundation Award nominee in health journalism, has been a widely supported nominee for the position of U.S. Surgeon General, and has received three honorary doctorates. Katz has served as a nutrition columnist for O, the Oprah Magazine; an on-air contributor for ABC News/Good Morning America, and with appearances on most major news programs and contributions to most major magazines and leading newspapers, including OpEds in the New York Times and Wall Street Journal.
His most recent book, How to Eat, co-authored with Mark Bittman, is a 2021 IACP Awards finalist and is worth your time. I have had the pleasure of hearing him speak many times over the years and he is an amazing orator and I am blessed to have this hour with him. Today, we dive into the science of childhood obesity and what are the public health pain points that can be pressed on to change the negative trajectory.

Sleep Update for 2024
Sleep has always been a necessary function of everyday life for all mammals. From an evolutionary perspective this cannot be a mistake as during the period of sleep you are vulnerable to predation. Therefore, there has to be a really good reason for mammals to sleep for extended hours. What is that reason? Why are toddlers and frankly all of us more cranky when we do not sleep enough? Why are our genes tied so tightly to circadian rhythms? Why are people so metabolically unwell when they are sleep deprived?

Dr. M’s Women and Children First Podcast #65 – Mark Houston, M.D. – Cardiovascular Health

Dr. Mark Houston is a thinker and researcher into the root causes of cardiovascular disease and metabolism. He graduated from Rhodes College in Memphis, Tennessee summa cum laude in Chemistry before graduating with honors from Vanderbilt Medical School. He completed his medical internship and residency at the University of California, San Francisco, then returned to Vanderbilt Medical Center where he was chief resident in medicine and served on the full- time faculty until 2012. He is the current director of the hypertension Institute where he and his team develop novel approaches to hypertension and ASCVD by attending to root biological causes of disease. He also has a Master’s degree in Human Nutrition from the University of Bridgeport, Connecticut, and a Masters of Science degree in Functional and Metabolic Medicine from the University of South Florida in Tampa Florida. He has written hundreds of papers, books and chapters on cardiovascular disease. He is one of the top researchers in the preventative cardiology space and he is here today to share his wisdom.

This week we talk all about sweat and heat liberation.

Sweating is a very important tool that the human body and most mammals use to eliminate unwanted stuff. In this case, the body primarily is eliminating heat and toxins via liquid sweat.
When we exercise vigorously or it is a hot day or we find ourselves overheating while fighting an infection, sweat will pour from our skin through glands and an endothermic reaction. The water that is released caries heat from the body with it. It evaporates into the air liberating the heat and thus cooling the body.
In this sweated liquid are electrolytes and toxins. Primarily, the sweat fluid has a natural release of small amounts of sodium and to an even lesser extent, potassium and calcium. This fluid will also facilitate the release of chemicals that the body does not like….. plus a few articles in a literature review.

Cholesterol VII- Follow up
It is imperative that all readers know that any time you go against conventional wisdom, you run the risk of negative reactions from providers of care that disagree or do not know about this depth of hypothetical disease etiology. This set of lipid and cardiology articles was meant to stimulate the reader to think beyond the simple answer that “statin medications” and cholesterol lowering are the primary or only answers to coronary artery heart disease. The treatment of any disease must start at the headwaters of pathophysiological dysfunction. To wait until a medicine is the only way to a healthy home is not in anyones best interest…. Also, the Pollution Detectives leader Dr. Francis Koster pens an article.
